● Parent or guardian ID
● Proof of residency - current utility bill (if doubling up and utility bill is under a different name other than yours, please submit a rent receipt or household owner verifying your residence
● Social security card
● Proof of income - current 4 paystubs or current SNAP letter
● Texas DFPS Placement Authorization Form (Form 2085) or court order that designated child is in the conservatorship of DFPS
● Proof of military status - if active/injured/casualty

To qualify, your child must meet at least one of the following:
